Photo and film consent (responsible person)
Consent form for photography/filming of a person when consent is given by a different person who is responsible for them.

As someone who is a guardian or has responsibility, I consent to Goldsmiths, University of London using photographs and/or video recordings including images of this person both internally and externally to promote the university. These images could be used in print and digital media formats including print publications, websites, e-marketing materials, posters banners, advertising, film, social media, teaching and research materials.
I understand that images on websites can be viewed throughout the world and not just in the United Kingdom and that some overseas countries may not provide the same level of protection to the rights of individuals as UK legislation provides.
I understand that Goldsmiths may continue to publish the images and recordings for up to four years. They may remain online and in the places where they were published after this date, and they may be kept as an archive of university life.
In completing this form I am also confirming if they are able to, the person in the image has given their own consent to having their photograph taken or to being filmed.
